Williamstown Middle/High School
WVBE Policy 2423, Health Promotion and Disease Prevention, includes the following health information for grades 7 and 12: proof of required immunizations for school enrollment; proof of the WV HealthCheck (well child visit) by a physician or advanced practice provider; and proof of a dental examination by a dentist. Proof of immunizations must be provided by first day of school or student will be excluded.
7th Grade Immunization Requirements
Must show proof of a dose of Tdap vaccine, which protects against tetanus, diphtheria, and pertussis (whooping cough) and a dose of the meningitis vaccine (MCV4).
12th Grade Immunization Requirements
Must show proof of 1 Tdap and 2 meningitis vaccines prior to starting school August. *If a student received their first meningococcal vaccine after turning age 16 then only 1 meningococcal vaccine is required.
Any student interested in playing sports must have a sports physical on file.
To be completed by physician following any hospitalization, emergency treatment, homebound instruction, extended illness, and/or nurse referral.
